the best way i have found to generate a test report is to first look at the new functionality utilized by the the updated version such as these and include these new features into an existing functionality report, if however you do not have an existing functionality test report (you're pre-live testing team should have this)
the tools we use are HP UFT to generate reports but there are also offerings from flexera aswell as the MTM from Microsoft.
it all depends on what you want the report to include
