Very straightforward InstallShield Setup with switch support.

Use setup.exe -r to record your setup. (If you make a mistake and have to use the back button, start over instead as the resulting setup.iss would not work). Find setup.iss in your %WinDir%.

To record the uninstall grab the uninstall string from the registry add a -r at the end and run it from the command line. Again you will find a setup.iss in your windows directory. I renamed mine to remove.iss afterwards.
